Bull NYU Hosp Jt 20 Dis. 20xx。 69(Suppl 1):S511. 综述 髋关节表面置换术的新进展 髋关节置换术 是 20 世纪骨科学中发展最为迅速的分支之一,尤其是常规全髋关节置换术,技术比较成熟,已在全世界范围内取得了非常瞩目的成就,改善了很多患者的髋关节功能。 但是对于年轻的髋关节疾病患者,常规全髋关节置换术则面临 着活动范围的受限以及较早的翻修术,使其难以满足患者的需求。 而随着材料及技术的发展,已经被大多数学者抛弃的髋关节表面置换术则再次进入专家的眼中,并且逐渐成为焦点。 历史回顾 髋关节表面置换不是一种新的概念,著名的 SmithPetersen 医生在 1948 年即建立了原 始模型,逐渐演变而形成我们今天所见的假体。 20 世纪 50 年代,著名的 Charnley 爵士设计了历史上第一种全髋关节表面置换假体,假体承重面材料是特氟隆( teflon) [1]。 这种假体出现了较高的早期失败率。 Charnley 当时认为股骨头缺血坏死是主要原因,知道后来应用于 THA 中才发现是材料本身不耐磨损的特性导致的。 20 世纪 60 年代, Townly 在承重面上采用了一种叫聚氨基甲酸乙酯的材料,其结果仍然是生产大量的磨损颗粒。 随后即被金属 聚乙烯假体取代。 1967 年, Muller 设计了一种金属 金属的关节假体,并对 18 名年轻患者进行了髋关节表面置换,尽管取得了良好的早起疗效,他仍然放弃了进一步的研究,转而使用金属 聚乙烯假体。 在随后的 25 年内,这类全金属假体中有 6。
